had the intake cleaning (crushed walnut shells)

these direct injection engines are really going to get gummed up if they're not taken care of --I don't myself know what the downstream issues would be, but this looks bad if left unchecked.

Did you notice any difference in drivability? I had a minor mis-fire issue before having mine done last month. After the cleaning its running much better.

acceleration in the midrange --i think i got accustomed to the way it was before. there was a low level stutter in the engine under light acceleration that is definitely gone now.

seems to only be an issue for early engine designs which co-opted non-DI flowpaths and bolted on DI fuel injection.

sounds like Ford, GM and Porsche might have figured this out and that 2nd gen Audi and BMW motors are less afflicted.

I've read it is much less of an issue in the N55, than the N54. Out of curiosity, how much were you charged? My mechanic flat rates at $500

I'm at 50,000 on my N54. Is there really any consensus here that this is something I should do prophylactically, with no obvious current issues or problems?

I don't think you have anything to worry about for 20-30k miles. Preventative? Not sure. I really wouldn't worry about it because it's so easy to take care of and it only takes half a day.
