The only two times this has happened to me, something was leaking. The first time it was the water pump on the e36 M3 and it was a slow leak that ate the belts too. The pump was covered by CPO, the belts were not.

The second time was on the e36 328 and it was a gasket of some kind. Hmm. No light that time. I noticed drips on the garage floor.

Come to think of it, just had a coolant leak on the M3. It was a hose.

I have not heard of as many E46 problems (but I haven't been following it that closely), but the E36 the radiator was a replace it around 10K or it will blow upi within about 10K.
